Sonographic features in cases with trisomy 18 and 13

OBJECTIVE: To evaluate the sonographic characteristics of fetuses with trisomy 18 and trisomy 13.
METHODS: From March 2002 to November 2011, we reviewed the database and medical records of 26 cases with trisomy 18 (n=21) and trisomy 13 (n=5). The subjects were recruited from pregnant women undergoing prenatal sonographic examinations at 13-28 weeks of gestation and subsequently proven trisomy 18 or 13. The results of ultrasound findings were reviewed in these cases with chromosomes confirmed as trisomy 18 or 13.
RESULTS: All cases had at least two abnormal sonographic finding. The common sonographic findings included choroid plexus cysts, abnormal head shape, cardiac anomalies, holoprosencephaly with associated facial anomalies, abnormal feet and/or hands, especially polydactyly, clenched hand and omphalocele. Non-structural abnormal findings such as polyhydroamnios or fetal growth restriction were seen in less than one third of the fetuses.
CONCLUSION: Nearly all fetuses with trisomy 18 or 13 had characteristic sonographic patterns of abnormalities demonstrated at midpregnancy. Detailed ultrasonography screening at midpregnancy is effective especially in fetuses with trisomy 18 or 13.

Trizomi 18 ve 13 olgularında ultrasonografi bulguları

AMAÇ: Trizomi 18 ve trizomi 13 tanısı almış fetuslara ait sonografik bulguları değerlendirmek.
YÖNTEMLER: Mart 2002 ile Kasım 2011 arasında hastanemizde saptanan 26 trizomi 18 ve 5 trizomi 13 olgusuna ait veriler ve tıbbi kayıtlar derlendi. Olgular 13-28. gebelik haftalarında prenatal ultrasonografik muayeneleri yapılan ve sonrasında trizomi 18 veya 13 olduğu kanıtlanmış gebe kadınlardan seçilmiştir. Kromozomal olarak trizomi 13 veya 18 olduğu kesinleşen bu olgulardaki ultrasonografi bulguları gözden geçirildi.
BULGULAR: Tüm olguların en az iki patolojik ultrason görüntüsü vardı. Sık gözlenen bulgular koroid pleksus kisti, patolojik kafa şekli, kardiyak patolojiler, holoprozensefali ve ilişkili yüz anomalileri, anormal el ve/veya ayak şekli, polidaktili, pençe el ve omfalosel idi. Yapısal olmayan polihidroamnios ve gelişme geriliği gibi patolojik bulgular olguların üçte birinden daha az bir kısmında saptandı.
SONUÇ: Trizomi 18 veya 13 olgularının hemen hemen tümünün gebeliğin ortalarında ortaya çıkan karakteristik anomaliler gösteren ultrason bulguları mevcuttu. Gebeliğin orta döneminde yapılacak ayrıntılı ultrasonografi, ileri genetik araştırma trizomi için 13 veya 18’li fetusların etkin bir şekilde taranmasını sağlayabilir.
