Diagnostic Value of the Systemic Immune-Inflammation Index in Diagnosing Acute Cholecystitis

INTRODUCTION: Acute cholecystitis (AC) is an acute inflammatory disease of the gallbladder. Although there are algorithms used today in the diagnosis of AC, there is still a need for inexpensive and fast diagnostic parameters. The systemic immune-inflammation index (SIII) is a novel prognostic indicator of systemic inflammation. In our study, the prognostic value of SIII in the differential diagnosis of AC was investigated.
METHODS: Our study was designed as a retrospective single-center study. The study was conducted with 150 patients who were admitted to the emergency department with abdominal pain and diagnosed with AC and a control group of 150 patients not diagnosed with AC.
RESULTS: In our results, the white blood cell, neutrophil, and C-reactive protein mean values were found to be statistically significantly higher in the study group than in the control group. Once the cutoff value was established at 743.92 (×109 /dL), the SII was found to have a sensitivity of 70% and a specificity of 59.2% in the diagnosis of AC. This assessment was also performed for neutrophil-to-lymphocyte ratio, and with a cutoff value of 2.91, it had a sensitivity of 62% and a specificity of 61.3%. There was no significant relationship between the Gangrenous cholecystitis (GC) and non-GC groups in terms of the diagnostic value of SIII.
DISCUSSION AND CONCLUSION: The present study found that the SIII is an index that can be used in the diagnosis of AC but be unsuccessful in distinguishing between GC and non-GC types.

Akut kolesistit tanısında sistemik immün inflamatuar indeksin tanısal değerliliği

GİRİŞ ve AMAÇ: Akut kolesistit (AK), safra kesesinin akut inflamatuar bir hastalığıdır. AK tanısında günümüzde kullanılan algoritmalar olmasına rağmen, halen ucuz ve hızlı tanısal parametrelere ihtiyaç duyulmaktadır. Sistemik İmmün inflamatuar indeks (SIII), sistemik inflamasyonun yeni bir prognostik göstergesidir. Çalışmamızda AK tanısında SIII'nin prognostik değerliliği araştırıldı.
YÖNTEM ve GEREÇLER: Çalışmamız retrospektif ve tek merkezli bir çalışmadır. Çalışma, acil servise karın ağrısı ile başvuran ve AK tanısı alan 150 hasta ve AK olmayan 150 kontrol grubu ile yürütüldü.
BULGULAR: Sonuçlarımızda, çalışma grubunda, kontrol grubuna göre lökosit, nötrofil ve CRP ortalama değerleri anlamlı yüksekti. AK tanısında, SIII'in 743,92 (x109 /dL) kesme değeri için duyarlılığı %70, özgüllüğü %59,2 olarak bulundu. Bu değerlendirme nötrofil-lenfosit oranı için de yapıldı ve 2.91 kesme değeri ile %62 duyarlılığa ve %61,3 özgüllüğe sahipti. SIII'ün tanı değeri açısından gangrenöz kolesistit olan ve olmayan gruplar arasında anlamlı bir ilişki yoktu.
TARTIŞMA ve SONUÇ: Bu çalışma, SIII'ün AK tanısında kullanılabilen ancak gangrenöz kolesistit ve non-gangrenöz kolesistit tiplerini ayırt etmede başarısız bir indeks olduğunu bulmuştur.
