INTRODUCTION: Perianal abscess (PA) and anal fistula (AF) are considered sequential manifestations of cryptoglandular infection. Long-term data on fistula formation and recurrence after incision and drainage (I&D) remain limited. Most published series use short follow-up periods. This study aimed to determine the long-term AF formation rate (primary outcome) and abscess recurrence rate (secondary outcome) after I&D for PA, using a cohort with a minimum follow-up of 10 years.
METHODS: This single-center retrospective cohort study included all consecutive adult patients who underwent I&D for PA between April 2014 and April 2016. Later events were ascertained by cross-referencing the institutional surgical database (2014–2026) with the Turkish national e-Health record system (e-Nabız), so fistula or recurrence episodes treated at other institutions could be captured. Patients with inflammatory bowel disease, colorectal ma-lignancy, prior perianal surgery, immunosuppression, or age under 18 years were excluded. The study was reported using the STROBE guidelines.
RESULTS: A total of 226 patients were included (145 male [64.2%]; mean age: 37.4 ± 12.0 years). AF developed in 54 patients (23.9%) at a median of 11.0 months (range: 1–105 months) after I&D. Transsphincteric fistulas were the most common (35/54, 64.8%). Abscess recurrence occurred in 53 patients (23.5%) (median: 9.1 months [range: 1.0–99.6 months]). Two outcomes overlapped in 29 patients. The observed fistula rate was consistent with published estimates (25–40%). The wide range of time-to-diagnosis (1–105 months) indicates that short follow-up may underestimate the cumulative incidence.
DISCUSSION AND CONCLUSION: Approximately one-quarter of patients developed AF, and a further quarter developed recurrent abscesses. Events occurred beyond the early postoperative period. Within the limits of a database-based design, these findings support counseling about the long-term risk of fistula formation and recurrence after I&D for PA.
