Multi-recurring extraplueural malign solitary fibrous tumor of breast: A case report and review of the literature

Introduction:
Extrapleural malign solitary fibrous tumor (EMSFT) is one of the soft tissue sarcoma group originating from the vascular pericytes of Zimmermann. Soft tissue sarcoma is rare and represents less than 1% of all malignancies in adulthood. A MSFT of the breast is even rarer. There are few cases reported in the literature. There is no enough data about the prognosis and local or distant recurrence of this disease.
Case: Here, we report a case of a 47-year-old woman who presented with recurrent masses in her thoracic wall and subsequent to her left breast. In 2010, she received primary diagnosis of MSFT histologically confirmed by core biopsy of a mass localized in subscapular region which extended to the left axilla. She underwent local excision with negative margins and received 60 Gy radiotherapy. She had four local recurrences and underwent excision within three years. Simultaneously, a new lesion occurred on the left breast during last recurrence. The histological examination of that lesion showed a primary MSFT of the breast. She underwent fourth local excision with negative margins and lumpectomy. She is under follow up.
Conclusion: Malign solitary fibrous tumor is a rare soft tissue tumor with high recurrence risk, which requires radical resection and close follow-up. Unfortunately adjuvant therapy modalities are ineffective for this disease.Multiple recurrences should be thought in these patients

Memenin çoklu nüks gösteren ekstraplevral malign soliter fibröz tümörü: Olgu sunumu ve literatür taraması

Giriş:
Ekstraplevral malign soliter fibröz tümörler (EMSFT) vasküler Zimmerman perisitlerinden kaynak alan yumuşak doku sarkomu gruplarından biridir. Yumuşak doku sarkomları nadirdir ve erişkin malignitelerinin %1’inden azını oluşturur. Hatta meme hemanjiyoperisitoması daha da nadirdir. Literatürde az sayıda olgu mevcuttur. Prognozu ve lokal veya uzak nüksüne ilişkin yeterli veri yoktur.

Olgu: 47 yaşındaki bayan hasta 2010 yılında, sol aksillaya uzanan skapula altı bölge yerleşimli kalın iğne biyopsisi histolojik olarak tanılanmış MSFT tanısını almıştır. Hastaya negatif sınırlı lokal eksizyon ve 60 Gy radyoterapi uygulanmış. Hastada 3 yıl içinde 4 lokal nüks gerçekleşmiş ve eksizyonu yapılmıştır. Son nüksünde eş zamanlı olarak sol memede yeni bir lezyon oluşmuştur. Bu lezyonun histolojik incelemesi MSFT olduğunu göstermiştir. Hastaya negatif sınır ile dördüncü lokal eksizyon ve lumpektomi yapılmıştır, ve hasta takip altındandır.
Sonuç:
Malign soliter fibröz tümör yüksek nüks riski olan, radikal cerrahi uygulanmasını ve yakın takibi gerektiren ancak adjuvan tedavinin etkin olamadığı nadir bir yumuşak doku sarkomudur.Çoklu nüks gelişimi bu hastaların takibinde öngörülmelidir.
