The Effect on Mortality and Morbidity of Neonatal Transport in Preterm Babies

OBJECTIVE: To investigate the effects of neonatal transportation of high risk preterm infants on morbidity prospectively
METHODS: Preterm infants between 24-37 gestational weeks younger than 7 days of age who were transported to Bakırköy Maternity and Children’s Hospital between May-December 2005 were our study group (Group l). The control group (Group ll) was included the same birth weight and gestational weeks preterms who were born in our hospital in the same period. Each group contained 50 preterm infants. The two groups were compared between maternal and perinatal risk factors, acute postnatal problems, neonatal morbidity and mortality rates. This study is a prospective nonrandomized controled study.
RESULTS: Demographic data of maternal age, delivery mode, gender, birth weight and gestational weeks were similar between the groups. Both acute problems such as hypothermia, hypoglycemia, acidosis and morbidities developed during follow up such as sepsis and intraventricular hemorrhages of the germinal matrix (GM-IVH) were significantly higher in Group l than Group ll (p<0,05). There was no difference between the groups in term of other neonatal morbidities such as respiratory distress syndrome (RDS), retinopathy of prematurity (ROP), necrotizing enterocholitis (NEC), bronchopulmonary displasia (BPD) and hyperbilirubinemia. The length of oxygen therapy and hospital stay were longer and mortality was higher in Group l than Group ll (p<0,05).
CONCLUSION: This study demonstrates that neonatal transportation increases the morbidity and mortality rates in high risk preterm infants.

Neonatal Transportun Preterm Bebeklerin Morbidite ve Mortalitesine Etkileri

AMAÇ: Neonatal transportun riskli preterm bebeklerin morbidite ve mortalitesi üzerine olan etkisini prospektif olarak araştırmaktı.
YÖNTEMLER: : Mayıs – Aralık 2005 tarihleri arasında hastanemize ilk 7 günde başka bir merkezden nakledilen 37 gebelik haftasından küçük pretermler ile hastanemizde doğan benzer gebelik haftası ve doğum ağırlıklı pretermler çeşitli parametreler açısından karşılaştırıldı. Bu parametreler; maternal ve perinatal risk faktörleri, akut postnatal sorunlar ve neonatal dönem morbiditeleri ve mortalite oranları idi.

BULGULAR: Transport edilen (çalışma grubu) Grup l, Hastanemizde doğan pretermler ( kontrol grubu) Grup ll olarak sınıflandırıldı. Her iki grup (27-37 gebelik haftası) peritanal, maternal risk faktörleri yönünden benzer özellikteydi. Gruplar arasındaki anne yaşı, doğum şekli, cinsiyet, doğum ağırlığı ve gebelik haftası benzerdi. Akut postnatal sorunlardan hipotermi, hipoglisemi ve asidoz Grup l’de anlamlı yüksekti. İzlem sırasında saptanan morbiditelerden; sepsis ve germinal matriks intraventiküler kanama (GMİVK) Grup l’de anlamlı yüksek bulunurken diğer neonatal morbiditeler bakımından gruplar arasında fark bulunmadı. Grup l’deki pretermlerde oksijen tedavi süresi ve hastanede yatış süresi uzundu ve mortalite yüksekti (p<0.05).
SONUÇ: Bu çalışma neonatal transportun; riskli pretermlerde, morbidite ve mortaliteyi artırdığını ortaya koymuştur.
